Safety Notice – Helmets Required

For the safety of all participants:

HELMETS ARE REQUIRED TO PARTICIPATE IN THIS BIKE RIDE.
No exceptions will be made. All riders must wear a properly fitted helmet during the entire duration of the event.

DEVELOPING K.I.D.S. BIKE RIDE FUNDRAISER WAIVER & RELEASE OF LIABILITY

Please review the information below carefully. By submitting this registration form, you acknowledge and agree to the terms outlined for participation in the Developing K.I.D.S. Bike Ride Fundraiser.

  • In consideration of being permitted to participate in the Developing KIDS Bike Ride Fundraiser, I hereby, for myself, my heirs, executors, administrators, and assigns, waive, release, and forever discharge Developing KIDS, its officers, directors, employees, volunteers, sponsors, partners, event organizers, and affiliates from any and all liability for any injuries, damages, losses, or claims (including death), whether known or unknown, arising from or in connection with my participation in this event.

  • I acknowledge that participating in a bicycle ride carries inherent risks, including but not limited to accidents, falls, collisions, weather conditions, road hazards, and equipment failure. I voluntarily assume all such risks, known and unknown, even if arising from the negligence of others.

  • I certify that I am physically fit, have sufficiently prepared or trained for participation in this event, and have not been advised otherwise by a qualified medical professional. I understand that medical support may not be immediately available during the event and that it is my responsibility to ensure I can safely complete the ride.

  • In addition, I understand and agree that Developing K.I.D.S., its Officers, Directors, Agents, and Representatives will and are hereby authorized to make audio and/or video recordings of Developing K.I.D.S.' programs and activities.  On behalf of myself and this child(ren), I hereby authorize Developing K.I.D.S. without payment to myself or on behalf of this child(ren) to record this child(ren)'s picture and voice on photographs, films, and tapes, to edit these recordings at its discretion and to incorporate these recordings into movie and sound films or tapes, broadcasts (radio/television) programs, world wide web based internet sites and home pages, public relations and advertising materials, or otherwise, and to license others to use the same in any manner of media whatsoever.  I will not hold Developing K.I.D.S. and their employees responsible for any damages resulting in the use of these publicity materials.

  • If the participant is under 18 years of age, a parent or legal guardian must also sign below.
